By default Pulse updates the documentation every early morning at 1:45 am, you can change this time in the instance settings:
If you want to know when was the documentation updated for the last time, you can see it in Model Spotlight, in the example below the documentation ran at 5pm:
If something is not working properly like Change Tracking, Documentation or Migration, a first thing to check would be to see if the documentation ran successfully.
